Michigan Choice Medicaid Waiver

MI Choice Medicaid Waiver (MI Choice) is a long-term care program. The program is available to certified disabled individuals (age 18 to 64) or individuals 65 years and over, who meet the eligibility requirements for nursing facility placement and are able to meet income and asset guidelines for Medicaid. MI Choice is available to those who live in their own home, the home of a loved one or in some licensed assisted living settings. A licensed Social Worker and Registered Nurse develop a plan for in-home care that is centered on an individual’s needs.

Services are arranged through a pool of provider organizations that are under strict guidelines and oversight. MI Choice Waiver Services May Include:

Program of All-Inclusive Care for the Elderly

PACE is a federal- and state-funded program throughout the country. The PACE model is centered on the belief that it is better for you and your family to be served in your own community whenever possible.

PACE uses Medicare and Medicaid funds to cover all of your medically necessary care and services. You can also pay for PACE privately if you don’t have Medicare or Medicaid.

PACE embraces your physical, mental, and social health in a “one-stop shopping” model with you at the center. Care is delivered at a full-service Primary Care Clinic, adult Day Center, and through a network of community providers.
